The 3441 form requires you to provide updated information about your medical conditions, doctors, medical treatment, medications, activities, work and education.  It also provides an opportunity to provide any other information you believe SSA should know about.  You should be very accurate and thorough when filling out this form.  If you fail to list all your doctors, SSA will not have all the information, and you increase your chances of getting denied again.

It is very helpful to submit opinion evidence from your doctors when you appeal.  This might include medical records or medical source statements.  A medical source statement is a letter or form filled out by your doctor which identifies any limitations you might have if you were placed in a full-time competitive work environment.
